Official “Personal Coaching with the Black Belt Coaches” Giveaway Rules

1. Eligibility

This giveaway is open only to attendees of the 12th Annual Dental Success Summit who are 18 years of age or older at the time of entry. Employees, contractors, and affiliates of MYCMOSHARE DIGITAL, LLC (dba CMOShare), as well as their immediate family members, are not eligible to participate.

2. How to Enter

To enter, participants must:

• Visit the CMOShare booth at the 12th Annual Dental Success Summit.
• Complete the entry form by filling out the form located here: [https://www.mycmoshare.com/dsi-summit-2025/].
• Entries must be received by June 14th, 2025 at 5pm.

Only one entry per person is allowed. Duplicate entries will be disqualified.

3. Prizes

There will be four (4) winners, each receiving one of the following prizes:

1. Visit to Dr. Addison Killeen’s office. See his operations, meet his staff and get a one on one day of coaching with him. Up to 2 people. 
2. Peak Dental Resource course. Get two free registrations for Dr. Ben Kacos’s Peak Dental Resource course of your choice. 
3. Visit to Dr. Bill Keith’s office. See his operations, meet his staff and get a one on one day of coaching with him. Up to 2 people. 
4. Jason Tanoory’s Servant Leadership course.

Prizes are non-transferable and no cash or other substitution is permitted except at the discretion of CMOShare.

4. Winner Selection and Notification

Winners will be selected in a random drawing conducted by CMOShare on or about [June 21st]. Winners will be notified via [email / phone / in-person] by [June 23rd at 5PM].

If a selected winner cannot be contacted or does not respond within 48 hours, an alternate winner may be chosen.

5. Publicity

By entering, participants grant CMOShare the right to use their name, likeness, and entry for promotional purposes without further compensation, unless prohibited by law.

6. Privacy

Personal information collected will be used solely for the purposes of this giveaway and in accordance with CMOShare’s Privacy Policy.

7. Limitation of Liability

CMOShare is not responsible for any incorrect or inaccurate information, technical errors, or unauthorized human intervention that may occur in the administration of the giveaway.
